Notes on completing the application form

1. CANDIDATE DETAILS

  • Title
    Delete as appropriate or insert your title.

     
  • Candidate number
    If you have already taken one or more of these examinations, you will have been allocated a candidate number. Please insert that number, as this will allow us to match your results with previous records. This is particularly important if you have previous passes. Failure to use the same candidate number may result in duplicate enrolments and delay in obtaining a certificate.

     
  • Address
    Insert the address (including post code) you wish your results to be sent to.

     
  • Telephone number
    Please give a daytime number we can use if we need to contact you.

2. EXAMINATION DETAILS

To qualify as a DGSA, you must pass three subjects as follows:

Core
One mode paper – Road or Rail or Inland Waterways
One class paper – All Classes or Class 1 or Class 2 or Class 3 or Class 7

You do not have to sit all three at one sitting, but you must achieve three passes within 12 months to qualify.

If you want to sit more than one mode paper on one date, please contact SQA on the number overleaf as soon as possible.

3. EXAMINATION CENTRE DETAILS

Please insert the examination centre you would like to sit at from the list provided. It would be helpful if you could give a choice of venues, with your preferred option first. Candidates should note that the availability of venues is dependent on the number of candidates. It is important that you check the centre address on your confirmation letter, as it may not be your first choice.

Examination Fees

Please enclose a cheque or postal order, payable to the SQA, for the appropriate fee.

  • £105 for one examination
  • £210 for two examinations
  • £285 for three examinations on one date

Unsigned applications and applications without the correct fee will not be accepted.

GENERAL NOTES

Organisations may wish to submit multiple applications on behalf of candidates. In this case, you should ensure that each candidate completes an application form and that the appropriate fee is submitted with the forms.

Refunds

Examination fees may be refunded or transferred to a later diet ONLY when you are prevented, by accident or illness, from attempting the examination(s) you applied for.  A fee of £10 will be charged for administration.

If you choose to transfer the fees to a later diet, you must attempt the examination(s) within six months of your original examination date.

Application for refunds or transfer of fees must be submitted with a medical certificate.

You are advised to wait for your results before applying to resit any examinations, as fees are non-refundable. Results will be issued approximately eight weeks after the examination date.
