Attendance Register Supplement forms

Ahead of the uplift of coursework materials on Thursday 16 March, please ensure that an Attendance Register Supplement form is completed for any candidate who is not listed on the Attendance Register (Ex6) forms supplied by SQA.

For these candidates, you must submit a new entry or change of level in the normal way before the associated uplift date.

If an entry is not processed for these candidates, it could result in data discrepancies between SQA’s database and your MIS system that may cause your centre further difficulties submitting data to SQA.

Please note that late entry charges will be incurred if entries are not submitted by Friday 31 March.
