Data management forms

The following is a list of forms which can be used by centres to submit candidate registrations; entries; entries updates; estimates; and internal assessment marks or grades. Select a form title to open.

Registration

Candidate Registration/Update Form - Schools CR00S (117 KB)
This form is used to register school candidates with SQA and can also be used to update registration details that we already hold on our system.

Candidate Date of Birth Amendment Form DBA1 (42 KB)
This form is the only means to Amend Candidate Date of Birth.  This change cannot be submitted electronically via your MIS system.

Entry

Entry Creation & Unit Result Form ECUR (23 KB)
This form is used to enter an individual/group of candidates for a qualification, ie courses, clusters and group awards where you are unable to submit the data electronically.
Enter and/or result an individual/group of candidates for units where you are unable to submit the data electronically. Please note that only units can be resulted.

Entry Update: Completion Date Change EU01a (92 KB)
This form is used to make updates to completion dates, for an individual or group of candidates for Courses, Units, Graded Units, Clusters or Group Awards.

Entry Update: Level Change EU01b (99 KB)
This form is used to make updates to course or unit level, for an individual or group of candidates for Courses, Units, Graded Units, Clusters or Group Awards.

Entry Update: Withdrawal EU01c (80 KB)
This form is used to withdraw individual or group of candidates for Courses, Units, Graded Units, Clusters or Group Awards.

External Examination- Alternative Centre Form SA00 (63 KB)
This form is more commonly known as the 'sitting at' form. It is used to let us know when a candidate is entered by one centre, but wishes to sit the examination at another SQA approved centre and enables us to send question papers to the 'sitting at' centre for the candidate.

National Courses

National Courses - Estimate Supplement Form (21 KB)
Used either when the information contained on the pre-printed Estimate Report is incorrect or if an entry has missed the deadline for the production of the Estimate Report.

Internal Assessment Mark Form – Supplement Form (43 KB)
This form should be used when a candidate does not appear on the pre-printed Internal Assessment Mark Form. You must ensure the correct entry information has been submitted, and that any marks on this form are not higher than the marks available for the component of the Course.
