Submitting Invigilator Report Forms

You must report all irregularities during an exam using an Invigilator Report Form. You can download the report form template from www.sqa.org.uk/examsupport.

You should ask the invigilator who observed the incident to complete the form. Please then ensure that your chief invigilator signs the completed Invigilator Report Form (a typed name is acceptable) and that you send it to invigilator.enquiries@sqa.org.uk within two days of the exam. Where reports refer to any physical evidence, please use the return envelope marked ‘Faulty Question Papers/CDs/Contaminated Scripts’ to return these to SQA. You must also give a copy of the report form to your head of centre or their delegate.

Chief invigilator feedback from 2024 was that 42% didn’t have easy access to a computer to submit Invigilator Report Forms. Please ensure your chief invigilator knows where to access a computer during their employment at your centre.
