The following pages are only for use by candidates to request replacement certificates or certified statements.
Certificates form part of an individual's personal data and are covered by the provisions of the Data Protection Act 1998. Under the terms of this Act, it is an offence to provide or obtain the personal data of another individual without their knowledge and express consent. Both SQA and the requester are required to observe the terms of this Act.
It is a condition of this service that you confirm that you are requesting a copy of the certificate or a certified statement relating to your awards and not those of any other person.
SQA reserve the right to refuse to process any request if the identity of the requester is in question. In this event, you will be contacted via the e-mail address with a request for such further clarification as SQA may consider necessary. In the event that SQA is not satisfied that the requester is the candidate whose data is requested, any payments made will be refunded.
